Vinfen Awarded $175,000 Grant to Expand Smartphone App to Adult Population

Screen Shot 2015-06-11 at 12.44.41 AMVinfen is thrilled to announce that thanks to an award from the Blue Cross Blue Shield of Massachusetts Foundation, our Smartphone app designed to enhance services to young adults with emotional and behavioral health challenges will be expanded to support adults with serious mental illness as part of project to improve integration and access to healthcare for this underserved population.

Vinfen was one of eight health care organizations to receive funding as part of the Foundation’s initiative – Fostering Effective Integration of Behavioral Health and Primary Care.  Through this initiative, the Blue Cross Blue Shield of Massachusetts Foundation will evaluate and assess experienced integrated care models that have the greatest likelihood of demonstrating the impact of integration on increased access, improved outcomes, and greater patient engagement and satisfaction.

“Coordinating access to both behavioral health care and primary health care is challenging, yet we’ve seen several innovative models that are breaking down the silos between these services and providing truly integrated care,” said Audrey Shelto, President of the Blue Cross Blue Shield of Massachusetts Foundation.  “We are excited to continue our work in this critical area and help sustain programs that will advance the understanding of what constitutes effective integration as well as the impact of that integration.”

Designed in collaboration with Wellframe, the Vinfen Smartphone app combines Wellframe’s innovative healthcare platform with Vinfen’s clinical content to help adults with Serious Mental Illness manage their health. The Smartphone app provides users with mobile access to physical and mental health information in written, video and audio form on a variety of key topics such as chronic disease management, nutrition, medications, stress management, coping strategies and healthy lifestyles. The app also provides access to the users’ personalized ‘tool box’ that contains their most effective coping strategies and problem-solving techniques.

The award of $175,000 per year for three years from the Blue Cross Blue Shield of Massachusetts Foundation enables Vinfen to not only expand the use of the Smartphone app to people served by our Community Based Flexible Supports team in Plymouth County, but to also embed additional Health Outreach Workers into the team. The additional staff will address gaps in the healthcare system by collaborating with providers to improve access to care and by coaching the people we serve to engage with, navigate and make effective use of the healthcare system.
