Vinfen nurses support people in the community where they live and work. Our nurses truly make a difference in the lives of the people we serve every day.
Our nurses work with people in residential group homes or as part of a community outreach team, which might meet someone in their own home, at a hospital or rehabilitation facility, or in the community.
Types of Nursing positions at Vinfen:
Clinical Care Manager (RN)
The Clinical Care Manager (CCM) provides care coordination and clinical management for MassHealth members with complex medical and behavioral health needs. The CCM is at the helm of organizing and coordinating resources and services in response to the enrollee’s healthcare needs across multiple settings and inclusive of both LTSS and Social Determinants of Health (SDH) needs.
Integrated Health Nurse (LPN)
The LPC Care Coordinator provides intensive care coordination and clinical care management for MassHealth members with complex medical and behavioral health needs who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. The LPN will partner with the Community Partner team and the clinical staff of enrollee’s ACO/MCO to promote integrated care and to ensure the quality and continuity of care.
Outreach Team Nurse (RN)
The Team Nurse promotes the full integration and functioning of the persons served that is needed to live a healthy life in the community. The Team Nurse coordinates access to and management of health care for the person receiving services in conjunction with Care Coordination entities and associated medical and behavioral health providers.
Program Nurse (RN)
The Program Nurse functions in a direct care nursing capacity and consults with other clinicians regarding health promotion and maintenance, medication administration, and acute and chronic medical/psychiatric crisis management for persons served.
