Outreach Programs

Adult Community Clinical Services (ACCS) is a comprehensive, clinically focused service anchored by a multi-disciplinary team that help people living with long-term mental health and substance use challenges recover.

The ACCS staff work as a team to provide community-based services seven days a week and clinical coverage 24 hours a day via an integrated team led by clinical staff. Team members provide clinical and rehabilitative interventions and other services including peer, family, and  housing support. The Vinfen team also provides employment services to support people in finding the jobs that they want through our Competitive Integrated Employment Services (CIES) and will help coordinate medical care through our Community Partners or One Care program.

With the permission from the person served, their family, friends, or other people who are important in the individual’s life will be welcome to join with us as we work on goals together. ACCS services are available to people who are eligible to receive support from the  Massachusetts Department of Mental Health.

STAFF SUPPORTS INDIVIDUALS TO BUILD THE LIFE THEY WANT

EXAMPLE GOALS INCLUDE:

  • Finding a home and learning to live independently
  • Developing skills to cope with stress and manage your symptoms
  • Improving your income and finances through benefits and work, including referral to CIES
  • Connecting with peer and social supports
  • Getting healthier by eating, sleeping, and exercising, and working with our medical care coordination services
  • Transportation
  • Understanding and taking medication
  • Addressing substance use challenges 

PROGRAM FOR ASSERTIVE COMMUNITY TREATMENT

Vinfen offers the evidenced-based practice of Program for Assertive Community Treatment (PACT) to people living in surrounding areas including Metro Boston, Essex North, Quincy, Plymouth, and on Cape Cod and the Islands, which provide effective rehabilitative and clinical services to people with mental health conditions. Each PACT person served works with a multidisciplinary team of specialists who provide highly individualized services in the areas of psychiatry, social work, nursing, peer support, and vocational rehabilitation – all to help people reduce or prevent psychiatric relapse and continue on their journey of recovery. 

PACT is available to individuals who are eligible to receive support from the Massachusetts Department of Mental Health or are referred and funded by a MassHealth or One Care provider. 

PACT TEAM MEMBERS PROVIDE THE FOLLOWING SERVICES

  • Provide and/or coordinate all medical, mental health, and general health care
  • Support in learning to manage mental health symptoms
  • Immediate crisis response
  • Medication prescribing and administration assistance
  • Supportive therapy
  • Treatment for people with coexisting substance use challenges
  • Help with learning skills to manage everyday life responsibilities
  • Help with learning skills to succeed in social situations
  • Employment services
  • Legal issues assistance
  • Peer support services
  • Recovery support
  • Family supports, education, and skill teaching

COMMUNITY SUPPORT PROGRAM (CSP) SERVICES

Vinfen offers an intensive, short-term case management program in the Merrimack Valley for adults with mental health conditions or substance use challenges.

Program services are provided by a community-based mobile worker supervised by a licensed behavioral health clinician. The goal of the CSP is to ensure that people can access essential clinical services that support improved health and promote mental health recovery. The CSP can provide the following supports:

THE CSP CAN PROVIDE THE FOLLOWING SUPPORTS

 

Arranging for medical, mental health, and addiction services

 

Coaching and support to improve daily living skills

 

Collaboration and communication with providers

 

Assistance applying for benefits, financial resources, housing, food, and health insurance

 

Transportation to and accompaniment to medical and behavioral health appointments

TO BE ELIGIBLE FOR THE COMMUNITY SUPPORT PROGRAM, A PERSON MUST

  • Have a serious mental health or substance use diagnosis
  • Meet at least one of the following criteria:
    a) Be referred by a Primary Care Provider (PCP)
    b) Have multiple Emergency Department or Emergency Service Program (ESP) encounters within the past 3 months
    c) Have been in an inpatient or 24-hour level of care facility within the past 6 months
    d) Have documented barriers accessing or using necessary medical or behavioral health services
  • Be at least 18 years of age or older
  • Have MassHealth (excluding MassHealth Limited)
